Obtaining Reports
Reports are available for pickup at Fire Station #1 Monday to Friday between 8:00am and 4:00pm excluding certain holidays. Please call 708-331-3123 prior to coming in as reports take time to look up and copy. Reports cost $20.00 each. At this time, the South Holland Fire Department only accepts cash or a cashier’s check. No personal checks will be accepted. For your convenience, fill out our Report Request Form and print it out.
What you will need:
Fire Report
- Date of the incident/call.
- Approximate time incident/call took place.
- Approximate location incident/call took place.
- A description of the incident that took place.
- Government issued ID
Ambulance Report
Ambulance Reports are medical records, and as such, are subject to State and Federal privacy laws. Only the patient may request a copy of a medical record. Most requests for records are submitted by mail, but records may also be obtained in person when the need is urgent. Older records take longer to obtain. For your convenience, fill out our Report Request Form and print it out.
- Date of the incident/call.
- Approximate time incident/call took place.
- Approximate location incident/call took place.
- A description of the incident that took place.
- Government issued ID.
- If the patient is a minor, a copy of the patients birth certificate.
- For a patient other than the person requesting the report,
- power of attorney – healthcare
- or written permission from the patient with notary
If requesting by mail:
- A letter requesting an Ambulance Report with the above information
- A copy of the power of attorney – healthcare OR written permission from the patient with notary
- A cashier’s check for the correct amount, payable to South Holland Fire Department
- A self-addressed, stamped envelope
